Implementation of a Pressure Ulcer Prevention and Management Care Pathway

Background

In early 2013 the Pressure Ulcer Steering Group at The Royal Berkshire NHS Foundation Trust (RBFT) chaired by the Associate Director, with representation from Clinicians, Dieticians, Occupational Therapists, Practice Educators and patient representation identified key areas where work was needed to reduce the numbers of patients developing avoidable pressure ulcers during their hospital stay. A number of work streams were initiated, one of these was the systematic review of documentation relating to pressure ulcer prevention and management.
