This case series describes a simplified and reproducible “anvil-through-the-stoma and out-the-side” technique for robotic Hartmann’s reversal. Six consecutive cases (2024–2025) were performed using the da Vinci Xi platform by a single colorectal surgeon. The technique allows anvil insertion through the stoma, avoids robotic undocking, preserves pneumoperitoneum, and streamlines anastomotic preparation.

Our experience demonstrates technical feasibility and safety, with potential benefits including reduced operative time, improved efficiency during adhesiolysis, and decreased risk associated with additional stapler crossings or loss of insufflation.
