The scientific evidence has demonstrated treatment plans for patients with periodontal compromised teeth that involves dental extractions, a immediate prosthesis and after time implants placement. Recently it has implemented a staged approach to provide a fixed restoration without a removable provisional phase.
The aim of this approach is to permit implant placement and to strategically keep residual teeth as abutments for a provisional fixed denture during the implant healing period.
At UNAM patient is 65 years old with hopeless dentition in the maxillary, seven residual teeth were prepared for a provisional fixed denture, later five implants SLA were placed and in the same time dental extractions in strategical sites, so then it is necessary to adjust the provisional restauration for only four teeth. After the implants has healed residual teeth are extracted and the sixth implant was placed, finally a second fixed provisional restauration is delivered to the patient but this one it is implant-supported by four implants. Once the last implant has healed, the final fixed prosthesis metal-ceramic was placed over six implants.
Advantages of the technique include no removable provisional prosthesis, preserved function and enhanced esthetics during the treatment.
